How to Acquire. In order to get this achievement Corvo Attano must complete all missions after the prologue, alerting/killing no one but key targets. As this is one of the more difficult accolades to collect, it is paramount that Corvo takes alternative paths to the targets and/or objectives of the side missions.
Do weepers count for ghost?
If either weeper in the sewers dies, it negates Clean Hands. Also, if either weeper sees you, that will negate Ghost and Shadow achievements. Just remember, even though there’s no end-of-mission statistics screen for this “mini mission”, it still counts for the achievements.

The game DOES count Granny Rags’s death against you if you let Slackjaw kill her. It doesn’t even note it as your kill in the end of mission stat screen. So, if you really want to get Clean Hands with no fuss then either avoid doing things for Granny Rags or use Bend Time to steal the key before booking it to the exit.
